CHF Telemanagement Program
The CHF Telemanagement program provides daily monitoring of vital signs for those with congestive heart failure (CHF). Analysis of information provided by the monitoring systems helps health care professionals design treatments that will reduce fatigue, shortness of breath, and tissue swelling to enhance energy level and sense of well-being.
What is Congestive Heart Failure?
Congestive heart failure occurs when the heart does not pump efficiently enough to meet the body’s needs.
When this happens, patients experience the following symptoms:
- Shortness of breath
- Persistent coughing or wheezing
- Fluid retention resulting in swelling of feet, ankles, or abdomen
- Tiredness
- Lack of appetite
- Nausea
- Confusion, impaired thinking
- Increased heart rate
If you have these symptoms, your doctor may suggest that you participate in Hunt Regional’s CHF Telemanagement program.
How are Vital Signs Monitored?
Following assessment by a Registered Nurse, you will receive scales and instructions for calling our dedicated CHF line daily to report your weight as well as other clinical markers. The telephone line is monitored daily by a licensed professional who will return your call if your condition warrants.
Based on your physician’s instruction, you may receive a visit from a Licensed or Registered Nurse to further assess your condition and instruct you in your care.
You may be offered the option of a HomMed™ Monitoring unit. This unit will remain in your home and will measure your weight, blood pressure, heart rate, and blood oxygen level daily. The information will be transmitted to the central station which is monitored daily.
Fluctuations in your clinical assessment will be reported to your physician when appropriate.
The monitoring unit measures:
- Weight
- Blood pressure
- Heart rate
- Blood oxygen level
- Temperature
- Customized indicators ordered by your doctor
